When we speak of autonomy we also have to define what our concept of autonomy is. There are some preconditions that have to be fulfilled in order to consider a statement of a patient an autonomous decision.&lt;br /&gt;&lt;br /&gt;Can we, in the case of the women presented here, assume that she is mentally capable to make an autonomous statement?&lt;br /&gt;&lt;br /&gt;Wouldn&amp;#039;t it be an omission not to preclude other medical reasons for her (presumably) depressive state (bearing in mind her medical history, comprising of diagnoses related to the brain).&lt;br /&gt;&lt;br /&gt;Furthermore, in the tradition of a palliative approach, shouldn&amp;#039;t we try to find out what the internal reason of the lady for her decision is? Could there be something that she needs and we are not aware of that? Of course, we will never find this out for the given case. But it is useful to embed the concept of autonomy in a context of many aspects that we have to keep in mind when dealing with end-of-life decisions of our patients.&lt;br /&gt;&lt;br /&gt;More thoughts are needed.</description>

Thanks a lot to Jacco for his introductionary remarks that will lead us to the isolation of the central problem(s) in this case.&lt;br /&gt;&lt;br /&gt;I would like to motivate other contributors to devote their reflexions the following aspects:&lt;br /&gt;&lt;br /&gt;* Clarification of medical background&lt;br /&gt;* Isolation of the central problem(s)&lt;br /&gt;* Relations between involved persons: Relation to team members and other affected persons&lt;br /&gt;* Developing of alternatives and possible solutions&lt;br /&gt;&lt;br /&gt;Please wait with moral reflexions until the respective topic will be opened in about two weeks.&lt;br /&gt;&lt;br /&gt;The reason for this somehow &amp;#039;artificial&amp;#039; division of medical and moral aspects is to finally realize, that both these aspects are intrinsically linked.&lt;br /&gt;&lt;br /&gt;To give you an example for the need for clarification of medical details think of the following question:&lt;br /&gt;&lt;br /&gt;&amp;quot;Are the preconditions for an autonomous decision still given when the patient is already dehydrated and maybe mentally impaired? What applies in the case of the lady we are talking of?&amp;quot;&lt;br /&gt;&lt;br /&gt;You see, the concrete situation is of a high relevance and the topic quickly becomes quite complex.&lt;br /&gt;&lt;br /&gt;During the Evaluation phase in the end there will be time to sort and weigh arguments and lines of argumentation.</description>

End of life decisions &lt;br /&gt;“versterven”&lt;br /&gt;&lt;br /&gt;A woman (*1952) is admitted in a rehabilitation centre in the Netherlands with progressive problems with balance and coordination. She has dysarthria, memory defects, vertigo, problems with swallowing she falls about 30 times a week. Her sight is impaired. She is very tired but sleeps well. &lt;br /&gt;&lt;br /&gt;Medical history:&lt;br /&gt;1969 astrocytome grade 2: surgery and radiotherapy&lt;br /&gt;2004 + 2005: CVA&lt;br /&gt;The patient has a partner since 1988, they live together and do not have children. Her partner has been at home for the last two years, he was burned-out. Patient is independent in her activities of daily living, but she is expected to need more help soon. Bulbar reflexes are slow and drip-feed is advised. Comprehensive testing gives the diagnosis radio-necrosis with a slow progressive course. &lt;br /&gt;&lt;br /&gt;The patient decided that she did not want to live her life like this anymore: she refused the intake of food, fluids and medication. She has told everyone to not resuscitate her and she does not want to be admitted to an hospital for further treatment. She chose for this method (“versterving” / “abnegation”) and didn’t express the wish for euthanasia. Her prognosis is maximum ten days, because of her bad condition. &lt;br /&gt;&lt;br /&gt;With this decision the indication for rehabilitation is expired. Patient can not go home because of the health problems of her partner and doesn’t want to be admitted to an hospital. &lt;br /&gt;&lt;br /&gt;Questions&lt;br /&gt;- Should the patient be informed about the option of Euthanasia?&lt;br /&gt;- May the decision of the patient be accepted in consideration of the consequences?&lt;br /&gt;&lt;br /&gt;&lt;br /&gt;&lt;br /&gt;I am curious about your opinions. What do you think about euthanasia, abnegation, suicide and natural death? What should the physician do, how to act? In case you have further questions or remarks, please do not hesitate to react!
